The game begins with journalist Miles Upshur, who arrives at Mount Massive Asylum, a psychiatric hospital in search of a scoop. To his surprise, Miles discovers that the hospital is unguarded and all entrances are closed.

Having entered the hospital through the window, the journalist sees all the horror happening in it: dismembered corpses are scattered everywhere, and crazy and mutilated patients wander along the corridors.
